Van Etten and colleagues (2025), recently published in JAMA Network, investigated whether the context in which a mild traumatic brain injury (mTBI) occurs impacts functional outcomes.

Authors conducted a cohort study including 567 veterans, including individuals with mTBI acquired in the context of a traumatic event compared with individuals with a mTBI acquired in the absence of a traumatic event.

Authors found that functional outcomes were significantly worse for individuals who acquired a mTBI in the context of a traumatic event. In particular, these individuals had greater post-concussive symptoms, greater post-traumatic stress disorder symptom severity and greater levels of disability compared with individuals with mTBI acquired in the absence of a traumatic event.

However, it should be noted that the poorer functional outcomes in the group who acquired mTBI during traumatic events may also be explained by the fact that this group had a significantly greater number of lifetime traumatic brain injuries compared with the group who acquired mTBI in a non-traumatic context. Therefore, the findings from this study should be interpreted with caution.

Future research is therefore needed to replicate these findings in groups who are matched on the quantity of prior traumatic brain injuries to determine whether the context in which a brain injury occurs does indeed impact functional outcomes.

Dr McCorry notes this was a retrospective study, and so the psychological effects are subject to recall bias, as was the recall of other life MTBIs; there can be many other factors related to the psychological well-being of the individual beyond the perceived injury circumstances that may influence the outcome… but a very interesting observation. An interesting addition question would be the importance of a legal claim in mTBI.
